What Is Hospice Home Care?
Hospice is special care, administered
by an interdisciplinary team, for patients with a life-limiting
disease, where no cure or further treatment is beneficial. The
focus is highly individualized, offering pain relief and providing
emotional, social, medical and spiritual support to the patient
and family or support group. Hospice care at home, humane and
compassionate, focuses on making the patient as comfortable as
possible in a familiar, supportive setting, permitting death with
dignity.
What Services Are Offered?
- An interdisciplinary team often consisting of physicians, nurses, social workers, home health aides, therapists, clergy and trained volunteers
- Pain management and symptom control
- Around-the-clock nursing care, when needed, or daily nursing visits when appropriate
- Personal care and bathing
- Nutritional support and dietary counseling
- Special therapies, including massage and physical therapy
- Coordination of care, including facilitation of communication between the caretakers, doctors, pharmacists, clergy and funeral directors
- Medications, medical supplies and other equipment
- Spiritual support for the patient and family or support group
- Respite care, offering the patient’s caregivers some time off
- Family conferences to share information on the course of the illness, express concerns and
feelings, and guidance on the next step
- Bereavement care to assist survivors during the grieving process
